Locum Senior Social Worker - Palliative Care, Mullingar

ASAP start - 6 month locum contract (initially)

They have an available opportunity for a Senior Grade Social Worker on the Palliative Care team, covering Longford and Westmeath, with excellent experience, skills and knowledge, who display and deliver their passion for working in Social Work environments.

THE RESPONSIBILITIES:

The overall responsibility within this role will be to

§To provide a comprehensive social work service as part of a multidisciplinary team in the community, where the focus is on the psychosocial aspects of patient and family care and bereavement support.
• To provide emotional and psychosocial support to the person who is terminally ill, both children and adults.
• To assess the bereavement needs of family and those close to the patient and to offer or arrange for appropriate intervention.
• To assess the concerns and resources of the patient, their family and/or significant others and formulate care plans in conjunction with the patient/family/carer and multidisciplinary team.
• To offer direct and indirect support and help to children coming to terms with the serious illness and/or death of a family member or friend. Direct work with children includes one to one work, family work or group work.
• To support parents in supporting their children to come to terms with the serious illness or death of a significant person in their lives.
• To work in conjunction with the multidisciplinary team and in particular with the established social work service to provide continuity of care and support for patients and their families.
• To provide information and advice on welfare benefits, community resources and support and advocate on behalf of patients and their families.
• To be a resource to the Multidisciplinary Team as regards the psychosocial aspects of the patient's life, that can impact on the patient's capacity to cope with their illness.
• To consult and liaise with other agencies such as hospitals, primary care providers, community based care providers and voluntary agencies as appropriate.
• Ensure the delivery of social work services in accordance with legislation, policies and procedures, guidelines and protocols.


THE REQUIREMENTS:

To be eligible for the role, you will need:

• A recognised qualification as a Social Worker.
• Full CORU Registration.
• Ability to adapt to changing work environment.
• Professional approach to work.
• Flexibility in work practice.
• Excellent communications skills with children and adults.
