TTM Healthcare are recruiting for our award-winning private client in Waterford. This Hospital caters to people across the South East, it provides comprehensive, patient-centred care and advanced surgery for chronic and complex medical conditions.
Job Purpose:
Provide effective, efficient and safe pharmaceutical care to all patients under their care, under the direction of the Director of Pharmacy. Identify and address individual patient care needs where appropriate. Advise and provide information to other healthcare professionals, including other pharmacists, to assist them in their care of the patient. Any other duties assigned to him/her by the Director of Pharmacy.
Key Requirements:
• Be a registered Pharmacist with the Pharmaceutical Society of Ireland (PSI).
• Provide a clinical pharmacy service; be an active member of the multidisciplinary team; provide drug information to medical, nursing and healthcare professionals and patients, carry out prescription review, patient education, and documentation of clinical interventions.
• Participate in the development and expansion of Clinical Pharmacy Services.
• Ensure best practise and compliance with agreed guidelines for the use of medications, developing and/or revising these, where assigned by Director of Pharmacy.
• Participate in and develop the antimicrobial stewardship programme.
• Participate in the medication safety programme, including the monitoring, reporting and investigation of adverse drug reactions, medication incidents and errors.
