Position Available : Patient Flow Nurse - Short Stay Community Hospital - South Dublin


The Background :

Our Client one of the largest independent Healthcare providers of quality nursing home care in Ireland is actively looking to recruit a full time Patient Flow Nurse working in the Transitional Care Unit of the Community Hospital based in South Dublin. This unit provides ongoing medical monitoring, rehabilitation or re-enablement to patients following an acute hospital stay. The Community Hospital is there to help patients with their expressed desire to make a speedy recovery and return home.

Overview of the role:

To promote and maintain the highest attainable standard of nursing care within the hospital. This role reports to and is responsible to the Director of Nursing.

Duties and Responsibilities

Ensure that all residents are treated with dignity, privacy and respect and ensuring they are treated as individuals.
Maintain professional standards in keeping with An Bord Altranais Code of Professional Conduct and have an understanding of all An Bord Altranais documents and their implications for nursing practice.
Establish a personal and meaningful relationship with all residents, relatives and visitors, ensuring appropriate support and comfort is always given, especially to the bereaved.
Ensure all Healthcare policies are known and adhered to.
Maintain clear, concise, and informative assessments and records within the hospital in accordance with legislation and standards.
Knowledge and awareness of HIQA standards and ability to participate in inspection process.
Ensure all admissions to the hospital are carried out correctly and all appropriate documentation completed.
Will be expected to attend ward handovers daily and weekly MDT meetings.
Be open to covering floor/ward as necessary.
Liaise with doctors about admissions, prescriptions and ensure admission paperwork is ready each day
Follow-up on patients outpatient appointments if necessary
To take responsibility for patient admissions/discharges etc when patient flow manager is on leave.
Familiarise yourself with the MSW in the hospitals and liaise with them as necessary.
Develop a rapport with bed managers in the hospitals and LPHNs in the community

What are the qualifications, skills and experienced required?

Registered on the Irish live register (NMBI). Must have active PIN.
Has clinical experience working in Gerontological Care or Residential Nursing Care
Working experience of HIQA Standards and Health Act Regulations 2013
Fluent in English language (verbal and written) and be eligible to work in Ireland
Strong communication and listening skills
Work well with a care team
Caring and compassionate, and be passionate about providing high quality person-centred care
Have a basic knowledge of excel spreadsheets
Have a good understanding of applications for home supports and NHSS/Fair deal
Have a good working knowledge of HSE areas
