TTM Healthcare are currently recruiting a Mental Health Nurse on behalf of their Dublin based client who provide Homeless and Drug services.
Contract: Permanent Full time working 39 hours per week
Hours: 7am to 9pm Monday to Sunday
Salary: €35000 - €4000 DOE
Reports to: Clinical Nurse Manager
The Role:
To proacatively deliver a high quality person centred care provision that meets the needs of the service users by:
• Promoting peoples rights and responsibilities.
• Working as an effective member of the team.
• Developing, in consultation with service users, flexible and achievable care plans that are person centred and adhere to agreed guidelines.
• Ensuring that a Recovery Approach is used when providing support to service users
• Ensuring a collaborative approach is used, with effective communication links with external professional groups e.g. GPs, CMHTs, Liaison Teams and a range of non statutory agencies etc. and to work as an effective member of any Multi-Disciplinary Team.
• Provide support to service users who are in crisis using the latest evidence informed practice such as STORM and ASSIST
• Manage medications in line with NMBI guidelines
• Onward referral for service users as required and relevant to their identified goals
• Effectively manage a caseload of clients, working collaboratively to assist them through their recovery by delivering psychosocial interventions grounded in latest evidence based practice.
• Undertake responsibility for clinical risk and assessment needs and the formulation and implementation of management plans
• Recognising indicators of deteriorating mental health, acting appropriately and both internally and externally as required.
• Ensuring effective care pathways are provided to each service user.
• Planning and facilitating of Appointments and clinics for service users based on identified needs.
• Enabling service users, through education and raising awareness, to manage factors that affect their mental health.
• Adhere to all Health and Safety and protocols relating to Covid 19 and any Infection Control guidelines.
