Staff Nurses Required on a full time permanent basis for a Residential Care Home based in Co. Louth

Our client a residential care home that caters for 108 people and provides a positive living experience for people in a place they can flourish and call home and provide a Respite Service on behalf of the HSE catering for people from County Louth. Also, working with the HSE to provide a Transition/Step Down service, which comprises of 34 beds. This service supports people transitioning from Hospital to home or to long-term care, who would otherwise have remained in hospital until returning home was suitable or when a long-term bed became available.

They focus on making the transition to the care home a positive experience by establishing a clear picture of how each person wants to live.


Role: Staff Nurse (Person Centred)

Reports to: The Person in Charge / Care Manager

Objectives:

• To assist in the planning, directing and delivery of high quality person centred care
• To assist in the promotion of residents' independence and wellbeing through the physical environment and in the general day to day activities within all households.
• To encourage positive social interactions between staff and residents at all times
• To demonstrate a feelings matter most approach to people adapting to changes in their lives
• To maintain a positive person-centred culture within the households.
• To see all behaviour and communication by residents as meaningful
• To recognise signs of well-being and ill being in residents and react accordingly.
• To work as part of a team ensuring the quality of service meets with H.I.Q.A Standards, Health and Safety Requirements, good hygiene practices, drug management procedures and Statement of Purpose and function and its policies and procedures and to ensure best practice of policies and protocols as devised by the Nursing and Midwives council of An Bord Altranais in accordance with legislation
• Establish and maintain relationships with residents that are based on respect and equality and that promote their independence

Responsibilities:

Organisation
• Accurately assess and document the needs of residents, with the emphasis on their strengths, abilities, qualities and feelings.
• Represent the facility in a professional and positive manner at all times and in all communications with residents, visitors and staff.
• Ensure safe administration of medication and treatments in compliance with H.I.Q.A Standards, with An Bord Altranais Guidelines on Medication Management and the facilities medication policy.
• Report any ill-health amongst residents and make appropriate requests for GP and allied health professional visits when necessary.
• Ensure that residents' care plans and all documentation is completed and maintained in conjunction with residents, relatives and other healthcare professionals and in accordance with H.I.Q.A Standards and the facilities policy and procedures.
• Assess all aspects of residents' needs and provide health supervision and nursing when required.
• Maintain professional development through attending and contributing towards staff training, supervisions and meetings.
• Comply with the facilities Health and Safety Policy and procedures at all times.
• Maintain confidentiality regarding all issues and information relating to residents and the company.


Functional:

• Undertake a comprehensive social and health care assessment on all new residents, and maintain ongoing assessment and review of other residents' needs.
• Maintain accurate records and ensure each resident has an individualized, person-centred care plan. Residents' care plans to be reviewed on a monthly basis or more frequently if changes occur, in accordance with H.I.Q.A Standards and company policy.
• Ensure a clear and concise handover is given to all staff at the changeover of each shift.
• Assist in the care of residents who are terminally ill or who have a progressive illness.
• To contribute to the protection of vulnerable adults from abuse.
• To answer assistance calls, telephones take and pass on accurate messages.
• Carry out regular checks on residents to ensure their health, safety and wellbeing.
• Ensure all equipment is clean and well maintained, complying with local and national infection control guidelines.
• To act in accordance with the An Bord Altranais code of conduct at all times.
• Ensure that all residents are receiving the highest standard of nursing and social wellbeing, which directly relates to care plans and the regularly reassessed needs of the residents.
• Record and report resident observations within care plans to ensure maintenance of accurate and up to date records.
• Report immediately to the manager or person in charge, any illness of an infectious nature or accident incurred by a resident, colleague, self or another.
• Follow security measures to ensure the safety and wellbeing of residents, visitors, belongings and the property as a whole.
• To maintain the highest standards of cleanliness and tidiness within your household at all times.
• To uphold the Values of your household.
• To carry out such other reasonable duties, appropriate to the role according to the needs of the residents and the needs of the service.

Financial:

Ensure all commodities used in and around the households are sensibly utilised by all staff - i.e. monitor the usage of incontinence aids, wipes, dressings, electricity etc.
Report misuse, damage or breakages of equipment, fixtures and fittings.

Staffing:

Provide support for new staff members and Volunteers and provide help and guidance where appropriate.
