TTM Healthcare are currently recruiting a Care Manager on behalf of our North Dublin based Client.
This service provides high quality safe and effective care in the home and community, for patients of all ages with varying degrees of ability.
Reports to: Managing Director
Contract: Maternity Cover, 1 year
Roster: Monday to Thursday Friday 9am to 5.30pm and Friday 9am to 5pm
Salary: Negotiable depending on experience.
This position is open to Nurses or Care Managers coming from a Healthcare Assistant background.

The role of Care Manager is:
• The day to day management of care to all community based clients of this service in North Dublin.
• Oversee the overall quality of service delivery to all clients of this service based in North Dublin.
• Engaging directly with the clients/family members.
• Engaging directly with Medical Social Workers, Public Health Nurses, Primary Care Teams
• Providing leadership to all Health Care Assistants, Care Supervisors and Care Co-ordinators
Requirements
• Previous experience as a Care Manager with healthcare background
• Qualifications in any of the following are desirable
· Health Care Management
· Business
· Risk Management
Experience: (Ideally 5+ years post qualification)
Professional Skills
• Management experience in a health care setting
• Excellent communication skills
• Excellent planning and organisational skills
• Ability to cope under pressure,
• Good negotiating skills, ability to positively influence people
• Hold a Full Driving License and have your own transport
Key Work Functions: (Please note this is not an exhaustive list).
• Management in excess of 50+of community based Health Care Assistants
• Point of contact for 100+ community based Clients and their families
• Carry out customer assessments and develop a customer care plan based on the identified needs in line with best practice
• Ensure all Customer Home Care Packages are fully covered on an on-going basis.
• Provide specific support and guidance to Health Care Assistants and Care Supervisors.
• Ensure the effective implementation of Care Policies, Procedures, Protocols and Guidance Documents.
• Ensure staff compliance with safeguarding systems.
• Ensure the successful implementation of quality control systems i.e. Audit.
• Accountable for the management of staff files including continues Professional Development of all staff
• Be responsible for their own continued professional development and keeping their knowledge and skills up to date in line with current best practice
• Have a strong knowledge of the requirements of commissioning bodies i.e.HSE.
• Liaise with all Health Care Professionals as required.
• Create and maintain administrative systems.
• Implement induction programs.
• Identify staff training needs and provide accordingly.
• Establish and maintain effective working relationships.
• Ensure that appropriate types of intervention take place to meet the customers' needs.
• Liaise with other agencies involved with the customer to ensure the provision of integrated services.
• Have an excellent working knowledge of Quality and Safety systems.
• Comply with all GDPR/Health & Safety Regulations / Fire Safety Regulations
