TTM Healthcare on behalf of our well established client are looking to recruit a Community Nurse Manager for the Cork Region
Purpose of the Post
The role of the Nurse Manager will be to support direct staff in the delivery of personalised care to their service user in their home and oversee the management and safety of same.
Role and Responsibilities
Ensure that the provision of a high standard of care to the Individual and families with our client is consistent with the mission, vision, values and strategic planning.
Meet with all new service users and ensure on-going liaison with individual and family/carers
Provide potential service user and/or their relatives/representatives with information on our client
Communicate with relatives of service user and with visitors as requested. Liaise with families regularly to ensure they are aware of mechanisms to communicate with our client
Provide leadership and guidance at all times, particularly at times of emergency to all staff members
Ensure Field visits are conducted with all staff members;
Maintain CMS system to measure Outcome / quality of life measures and provide weekly monitoring reports and monthly commissioners reports
Ensure TMS is used properly within all homes Ensure roster anomalies are maintained
Become part of the on call team and become part of the rotation team. Demonstrate behaviour consistent with the Values of our client
Conduct environmental and audit checklists and reviews to ensure adherence to best practice JCI standards and HSE Tender Agreements and preparation of reports and analysis.
Monitor staff to oversee the delivery of individual care together with all members of the multi-disciplinary teams
Provide ongoing Assessment as required. Carry out first and second prior to undertaking of care. Pre-(Admission) Assessments (on Admission and after returning from Hospital,) Pre discharge / Pre contract as directed by Director of Services Advanced Homecare.
Plan, assess and deliver individualised care, together with all members of the multidisciplinary care team.
Provision of induction training for newly recruited staff. Participate with the orientation of all new staff to our client
Co-ordinate the quality and safety system including all aspects of clinical risk management and individual involvement using audit management tools.
Undertake counselling and appraisal of trained staff as required.
Keep abreast of developments in health and social care.
Professional supervision. Be responsible for the development, education and supervision of staff.
Facilitate the arrangements necessary and participate where appropriate, in education and training of other staff as the need arises.
Be responsible for the development and implementation of policies and procedures as directed by the Director of Services Advanced Homecare
Applicants must have at least 5 years post graduate Experience , 2 of which must be at management level
Knowledge of Organisation of health services and trends nationally and internationally
Demonstrate sound knowledge and social model practice together with future trends in support of individuals.
