TTM Healthcare Solutions are currently recruiting for both Staff and Senior grade Medical Social Workers for a HSE Hospital in Naas, Co. Kildare. This is an appointment to the Naas General Hospital setting, with service delivery in both Naas Hospital and Community/Primary Care sites by way of outreach clinics, to reflect the regional remit of the designated Orthopaedic and Rheumatology services.

You will work closely with:

•Senior Medical Social Worker
•Members of the Medical Social Work Team
•Hospital General Medical, Surgical and Geriatric Teams
•HSCP Teams
•Hospital Senior Management as appropriate
•Nursing staff across a wide range of specialties

Your role is to provide psychosocial and practical support to patients and their families who attend the hospital and are admitted for inpatient medical treatment. The successful candidate will be responsible for providing a generic Social work service and is expected to consult with the multidisciplinary team in coordinating safe and effective discharge plans on behalf of patients.

Duties and Responsibilities:

•Carry out psychosocial assessments with patients and families
•Facilitate discharge planning with patients where appropriate and liaise with community resources/services.
•Support patients in relation to the emotional and practical aspects of their illness and, where appropriate, advocate on their behalf.
•Respond appropriately and efficiently to referrals involving crisis, risk and statutory reporting.
•Work closely with the Multidisciplinary team to achieve common goals pertaining to patients.
•Effectively manage a busy caseload across multiple clinical divisions including surgery, medicine and the Emergency Department.
•Attend and contribute to weekly multidisciplinary meetings.
•Ensure the necessary liaison and co-operation with Medical, Nursing and other disciplines in the Hospital and in consultation with them, to arrange for case conferences in appropriate cases.
•Work with patient/families to implement the Care Plan in an effective and timely manner
•Keep and maintain such written and electronic records (including statistics) as are part of the structure/reporting of the Social Work Department.

You will be:

•Hold a recognised post graduate professional qualification in Social Work, NSWQ or equivalent.
•Be registered as a social worker with CORU (registration number to be provided on CV)
•Possess knowledge of Children's First legislation and best practice in Safeguarding Vulnerable Adults.
•Possess the clinical and administrative skills to fulfil the functions of the role.
•Ability to provide a direct Social Work service (compatible with the Hospital's function) to the patients and/or their relatives as assigned to him/her at the discretion of the Senior Social Worker.
•Ability to complete timely psychosocial assessment of patients and execute a formulated care plan with the multidisciplinary team.
•Experience of work in a multidisciplinary content, preferably a Hospital setting.(This is essential for Senior Post)
•Experience in carrying out psychosocial assessments with individuals and families and knowledge of safeguarding and child protection responsibilities.
