Locum Medical Social Worker - Phoenix Park, Dublin 20

ASAP start, 3-month contract initially (extension likely)


THE BACKGROUND:

At TTM Healthcare we are proud to have partnered with St. Mary's Hospital in Phoenix Park, Dublin - a dedicated campus that provides health care services to older adults and adults with disabilities. Services include residential services for older persons, frail elderly rehabilitation, specialist stroke rehabilitation, community response model of inpatient care and a Day Hospital service.

St. Mary's Hospital have an available opportunity for a Medical Social Worker with excellent experience, skills and knowledge, who displays and delivers their passion for working in Social Work environments to participate as a member of a Social Work Team, working as part of a multi-disciplinary team delivery a co-ordinated approach to client care.

THE RESPONSIBILITIES:

The overall responsibility within this role will be to

• Provide a social work service that seeks to improve the health and wellbeing of the service users within the context of their admission to St. Mary's Hospital.
• Work in partnership with patients and families to empower and enable coping with the impact of illness, disability, hospital admission and discharge.
• Achieve the above through the implementation of an accessible, equitable, person-centred professional social work service.
• Play a key role in co-ordinating community-based living services to ensure a safe and timely discharge from hospital back to the community.
• In the event that an older person cannot return to community living, the social worker is the key professional providing the older person and their family with information and support in applying for the nursing home support scheme. (NHSS)


THE REQUIREMENTS:

To be eligible for the role, you will need:

• A recognised qualification as a Social Worker.
• Full CORU Registration.
• Ability to adapt to changing work environment.
• Professional approach to work.
• Flexibility in work practice.
• Excellent communications skills with children and adults.
• Demonstrates an ability to assess psychosocial factors impacting on and frail older person
• Demonstrate an understanding of the impact of illness, hospitalisation and disability on a frail older person and their family
• Demonstrate knowledge of policy, legislation and best practice guidelines relevant to social work practice issues.
• Demonstrate ability to use theoretical evidence to support all work undertaken.
• Demonstrate a strong awareness of social work values.
• Demonstrate the ability to manage self in a busy working environment including the ability to prioritise caseloads.
• ability to form collaborative working relationships with colleagues internal and external to the hospital in order to promote the highest standards of care and service delivery
• Demonstrate ability to utilise supervision effectively including attending to ethical issues.
• Display awareness and appreciation of the service user as expert through experience including promoting the involvement of the service user in care planning, decision-making and service development.
• Demonstrate ability to treat service user, their families and all colleagues with dignity and respect.
