THE OPPORTUNITY:
Primary Care Locum Social Workers - Midlands counties, Westmeath/Longford/Laois/Offaly

ASAP starts, 6 month contracts initially

Base flexible - ideally Abbeyleix, Co. Laoise or Mullingar, Co. Westmeath

Social Work in Primary Care provides a Social Work service to the clients of the Primary Care services in the Laois/Offaly and Longford/Westmeath area, as part of the Primary Care multidisciplinary team.

Primary Care offers a generic cradle to grave service for clients in the Laois/Offaly and Longford/Westmeath areas, assisting people to deal with difficulties, access services and develop community supports.

THE RESPONSIBILITIES:

The overall responsibility within this role will be to

• Provide a direct point of access for the local community/designated service area and offer initial and comprehensive psychosocial social work assessments and develop intervention programmes to the individuals, families and groups.
• Provide an initial and comprehensive psychosocial Social Work assessment as appropriate to service users of Primary Care Teams.
• Identify service user' individual and collective needs in partnership with them and co-create interventions/care plans and/or social action strategies to meet those needs.
• Attend case conferences, departmental meetings and all relevant meetings as appropriate.
• Attend court, tribunals, etc. as required.
• Develop, mentor and evaluate outcomes of person-centred care plans for individual service users, families and communities.
• Work within a key worker / case worker system, providing a co-ordinating role for case management where appropriate.
• Assess where social conditions are a major factor in health and social wellbeing, consult and plan with the service user/relevant team/service and arrange appropriate social services for those who need them.
• Participate and take leadership in community needs assessment and on-going community involvement/intervention including initiating and participating in prevention and health promotion activities.

THE REQUIREMENTS:

To be eligible for the role, you will need:

• A recognised qualification as a Social Worker.
• Full CORU registration.
• Ability to adapt to changing work environment.
• Professional approach to work
• Flexibility in work practice
• Ability to demonstrate sufficient professional knowledge to carry out the duties and responsibilities of the role.
• Demonstrate the ability to manage self in un a busy working environment including the ability to prioritise caseloads.
• Excellent communications skills with children and adults.
• Access to a car and full drivers' licence.
