Our client is looking for a Clinical Nurse Specialist is to provide Heart Failure Care Services for the care and on-going management of patients with heart failure. The Successful candidate will be working across three hospitals on a weekly basis and have the flexibility to arrange their case load.
This post holder will be required to work as a key member of the multi-disciplinary team in the hospital providing physical, psychological, and emotional support to Heart Failure patients and their families throughout their disease trajectory.
The act as a liaison between Heart Failure services, Primary Care Teams, Community Care Teams, and other agencies. The post holder will work as part of the multi-disciplinary team to achieve the strategic objectives of the Heart Failure Service in hospitals nationally. The CNS will have direct involvement in and extensive knowledge of the specialist area of practice.

Duties and Responsibility
The CNS will be responsible for providing clinical advice and support for patients and their families and for other members of the multi-disciplinary team. The CNS will on occasions be required to make variations in prescribed clinical options, within agreed protocols. Speciality practice of the CNS includes:
• Clinical Practice.
• Education and Training.
• Research implementation and audit.
• Consultancy roles.
• Advocate.

Qualifications

• Registered with An Bord Altranais, on the general division of the live register.
• Five years post registration experience with recent involvement in an area related to the speciality.
• Evidence of further education. Appropriate expertise in the speciality and demonstration of advance clinical practice.
• Higher diploma in Cardiology Nursing / Critical Care Nursing.
• Proven interpersonal and organisational skills.
• Evidence of commitment to professional development, training and evidence base practice.
• Ability to perform independent nursing and recommend action plans.
• Organisational skills are important to allow for the provision of structure and continuity to individual patient therapy including priority setting and time.
• To fully understand and adhere to the programme SOP
• Preferably Level 9 professional qualification in Heart Failure Nursing
